Make a problem reproducible

Include the tool or guide page address and the action you tried.
Describe what you expected and what actually happened. Mention any displayed error.
Include your browser and device, and whether you tested a file, a screen, or a physical print.
If possible, provide a non-sensitive sample value that reproduces the problem. Remove passwords, private contact details, and protected-link credentials.

Before reporting a scan failure

A code that does not decode is a different problem from a correctly decoded URL that asks for a sign-in. The scan troubleshooting guide helps separate those cases. We cannot change a third-party destination’s sharing permissions or recover local projects after browser data has been cleared.
